Abstrak/Abstract Background: Currently the number of dentists in Indonesia has reached 23,888 with a ratio of 8 dentists per 100,000 population. The Ministry of Health has a target ratio of dentists in 2025 of 14 dentists per 100,000 population, so the number of dentists in Indonesia has not yet reached the target. On the other hand, Indonesia's Basic Health Research reported that the prevalence of caries in 2018 was 88.8%. Indonesia has implemented the National Health Insurance system since 2014, but it has not had a significant impact on oral health and utilization of oral health services is still relatively low. The utilization rate for oral health services at the primary care of the NHI system in November 2021 was only 4.87‰. The low utilization rate occurs partly due to the limited benefits of primary dental services and the benefits covered under NHI system do not reflect all the competencies of primary dentists. The benefits of primary dental services in NHI system currently still reflect treatment procedures, not based on coding diagnoses as stated in dentists’ clinical practice guidelines. Seeing also the limitations of primary health care, so that in practice primary dentists may not necessarily be able to carry out CPG optimally. Therefore, to improve oral health, it is necessary to review dental health services in the NHI era and review the implementation of CPG in primary health care which will produce outputs in the form of recommendations for improving CPG for dentists. Methods: A mixed-method approach with an explanatory sequential design will be used in this study. This design starts with the collection and analysis of quantitative data. This first phase is followed by the subsequent collection and analysis of qualitative data. The second, qualitative phase of the study is designed so that it follows from the results of the first, quantitative phase. Within this study, quantitative data to explore the competence of the primary care dentists based on the existing clinical practice guidelines will be collected as well as the data of dental infrastructure readiness to serve as the basis to review the content of the guidelines. Furthermore, secondary quantitative data from the Indonesian Social Security of Health (BPJS Kesehatan) will also be collected to describe the outcomes of oral disease cases, treatments as well as oral health practices. A qualitative method in the form of Focused Group Discussion (FGD) involving experts of each dental clinical field will follow to review the quantitative result. Expected result: Indonesia's dental and oral health status in the NHI era and obtaining dentist competency mapping in the primary health care, according to the level of competence related to capabilities, facilities and infrastructure in the primary health care. Recommendation: Policy makers obtain an overview of the dental health service system in NHI and recommendation items for improving clinical practice guidelines for dentists, especially primary dentists in NHI implementation.
