Background: A neglected fracture is a fracture that does not receive adequate treatment resulting in bone displacement and changes in muscle pull which complicate repositioning. The use of a mandibular 3D model can help decide the alignment of the normal mandible, and manufacture of an acrylic plate to fix the position of the mandible Purpose: This case report shows mandibular 3d models and acrylic plates in the repositioning process of neglected mandibular fracture. Case: A 3-year-old boy was referred to Dr. Sardjito General Hospital because the lower jaw was tilted to the left. According to his parents, the patient fell with his face stuck on the floor at the time of birth. Extra-oral objective examination showing mandible deviation to the left, palpation of the mandibular condyle in the anterior tragus is not palpated when opening and closing the mouth. Intraoral examination there is a step in the angle of left mandibular region. CT scan features mandibular angular fractures with a gap of 1 cm. Based on the examination results, the diagnosis is neglected fracture of mandibular so that reconstruction operations are needed. The repositioning and reduction of the fractured area during surgery are performed using acrylic plates and mandibular 3D models that made preoperatively. After six months, the results of the operation showed symmetrical facial features and the results of the panoramic X-ray examination show the position of the mandibular plate on angulus is good and has visible bone healing. Conclusion: Repositioning using mandibular 3D models and acrylic plates can help reposition, increase the accuracy of plate placement, and speed up operating time. |
