Abstrak/Abstract |
Context: Diabetes mellitus is one of the biggest non-communicable disease problems worldwide and 90% of diabetes cases are type 2 diabetes mellitus.
This chronic disease requires continuous monitoring by the interprofessional relations team to prevent the progression of more severe complications
and improve the quality of life.
Aims: To determine the models of interprofessional relations with patients with type 2 diabetes mellitus in the community and hospital.
Methods: A systematic literature search was conducted in three online databases (Google Scholar, Science Direct, and Scopus). From 22690 returned
studies, 12 met the predefined inclusion criteria. The quality of each paper was evaluated using the evidence-based practice tools provided by Johns
Hopkins. A PRISMA flow chart documented the search, exclusion, and inclusion criteria study.
Results: The majority of approaches employed comprised a mixture of training, daily huddles, coordinated care plans, joint patient visits, follow-up phone
calls with patients, medication reviews, educational initiatives, diabetic group sessions led by students, participation in scientific conferences, and
utilization of the care networks. Positive advancements were observed in HbA1c levels, blood pressure management, body mass index, low-density
lipoprotein cholesterol, total cholesterol, enhanced teamwork, data security, time efficiency, and automated processes.
Conclusion: Interprofessional relations with one or a combination of interventions positively affect clinical and non-clinical parameters in the community
and hospital. |
