Ensuring Sustainability of Polio Immunization in Health System Transition: Lessons from Polio Eradication Initiative in Indonesia

Background: Sustaining an effective evidence-based health intervention will maximize its impact in public health. Political and governmental reforms impact immunization program sustainability both positively and negatively. This study aims to explore sustainability of polio immunization in decentralized health system taking lesson learned from polio eradication initiative in Indonesia.
Methods: We collected qualitative through in-depth interviews with 27 polio key informants in Indonesia. The participants were from various background at district, province, and national level, consisting of frontline worker, manager and NGO at national and sub national level. We conducted thematic to analyze the data. Document reviews were conducted for triangulation. Member checking and peer debriefing were also conducted to ensure the validity.
Results: Competing priority was identified as the major challenge to sustain government commitment for polio immunization and AFP surveillance in decentralized health system transition. Moreover, the variation of capacity of local government has also affected the immunization delivery and the commitment at sub-national level government. The government reform also led to more democratic system thus more rejection and vaccine hesitancy were found. Multi sector partnership has been identified to have big role in maintaining polio immunization coverage. Strong and continuous advocacy and socialization are essential to raise awareness of community and policy makers to maintain the high polio immunization coverage.
Conclusion: Competing priority was the major factor that affect high coverage of polio immunization during the decentralization transition. Strong advocacy is needed at every level, from district to national level to keep polio immunization prioritized.
